Hillcroft’s Home & Community Based Services

Provides support to people with disabilities in a variety of settings. Supports include respite care, 24-hour supported and group living sites and scheduled drop-in services. Hillcroft’s Home & Community Services provides a well-trained group of professionals who offer children and adults with disabilities assistance and training to increase independent community living skills.

Supported Living Services

Adults with disabilities, as those without disabilities, often have the desire to live independently in their own home. Supported Living provides the assistance needed to live in the community, outside of a family home or long-term care facility.

Beyond providing basic support to live in a house or apartment, Supported Living Services provide participants with opportunities to develop skills in activities of daily living and to fully engage in their community. Services are developed to meet to the participant’s needs and may be provided for a few hours per week up to 24-hours a day.

Supported Living Services are developed to meet to the participant’s needs and may be provided for a few hours per week up to 24-hours a day.  Hillcroft’s Supported Living Services are coordinated by Home & Community Program Managers and are provided by direct support professionals (DSP) who receive initial and ongoing training including:

  • CPR and First Aid,
  • Medication Administration,
  • Prevention of Abuse and Neglect,
  • Positive Behavior Supports,
  • Aggression Management,
  • Individualized participant training,
  • 24-Hours of job shadowing with a previously trained staff member,
  • And quarterly Behavior Management training for staff who provide support to participants receiving Behavior Support Services.

Hillcroft’s Supported Living Services are offered through the Medicaid Waiver program. Hillcroft does not maintain a waiting list for these services as individuals with Medicaid Waiver funding are considered for situations that would be appropriate and would meet their needs. In non-emergency situations, Hillcroft is able to begin providing Supported Living Services within two weeks of an approved Medicaid Waiver budget.  Services may be able to begin immediately in emergency situations.

Creativity Unlimited

Hillcroft’s Creativity Unlimited Program (CU) supports participants in positive learning of everyday activities.

CU services are tailored to fit participants’ needs. Areas of focus include:

  • Driver Education
  • Daily Living Skills
  • Communication
  • Community Living
  • Functional Academics
  • Health and Safety
  • Leisure/Hobbies
  • Mobility
  • Advocacy
  • Personal Care
  • Nutrition Education
  • Personal Finance
  • Relationship Building
  • Personal Care
  • Sensory-Motor Development
  • Reverse Integration
  • Activities to Prevent Mental Decline
  • Stretching/Exercise

CU Services are provided by Direct Support Professionals who receive initial and ongoing training in CPR/First Aid, Universal Precautions, Medication Administration, Positive Behavior Supports, Preventing Abuse and Neglect, and Aggression Management Techniques. Each direct support professional in CU is also required to receive specific training on any individual with whom they work. These trainings are also updated per need/change in client status.

Creativity Unlimited Services are available through Medicaid Waiver funding or private pay. Group services are provided at staff ratios depending on the best needs of each client. In order to obtain Medicaid Waiver funding, an individual must contact the Bureau of Developmental Disability Services at 765-288-6516 to determine their eligibility. There currently is a waiting list for new Medicaid Waiver funding services through the Bureau of Developmental Disability Services.  CU programming can begin within two weeks of an approved Medicaid Waiver Notice of Action (NOA).

Hillcroft’s Thursday Night Rec program is available to adults with disabilities ages 18 and older. This program offers a variety of opportunities for individuals with disabilities to actively engage in recreation experiences.

Wellness Coordination

Our nursing department maintains Wellness and Health Care Coordination for individuals in Home & Community services programs, including Medicaid Waiver programs.

Through the direction of an RN/Nursing Supervisor, the Nurses and the department coordinate all Wellness and health care needs of the clients. This service includes oversight of services beyond routine doctor/health care visits.

Wellness Coordination includes the oversight and development of a Wellness Coordination Plan, coordination and training of staff on client’s health care needs, consultation with Physicians, Specialists, and other medical personnel, face-to-face consultations with the clients, including quarterly and annual physical assessments, consultation with support team members and active involvement in team meetings.

Respite Services

Individuals providing full-time support to children and adults with disabilities occasionally need a respite, or time away, to attend to other matters. Whether time is needed to run errands, take a vacation or attend appointments, Hillcroft’s Respite Services provide trained staff on a short-term basis to support participants while the primary caregiver is unavailable. Respite Services are typically provided in the family’s home but may also take place in community settings.

Hillcroft’s Respite Services are coordinated by Home & Community Program Supervisors and are provided by direct support professionals (DSP’s) who receive initial and ongoing training including:
